 紛争や災害などで家を追われた人、追われないまでも困難な状況で地元にとどまっている人へのHIV関連サービスの提供は、エイズ対策の大きな課題の一つです。国連合同エイズ計画(UNAIDS)は3月3日、共同スポンサー機関である国連難民高等弁務官事務所UNHCR)および世界食糧計画(WFP)と共催で「HIVと人道上の緊急事態」をテーマに専門家会合を開きました。その簡単な報告の日本語仮訳をHATプロジェクトのブログに掲載しました。

 今年7月の国連合同エイズ計画(UNAIDS)プログラム調整理事会では、2030年のエイズ流行終結を目指すうえで不可欠な課題として、この問題が改めて集中討議されるようです。今回の会合については、議論の内容はあまり立ち入って紹介されていません。関係者コメントだけでお茶をにごしている印象です。7月に向けた下準備といったところでしょうか。

 UNAIDSとしては、2030年のエイズ流行終結という大目標(ただし、「公衆衛生の脅威としての流行」の終結という逃げ道は残しています)に向けて今後5年間にどこまで対策を強化できるかが勝負ととらえています。そのために、10代の若者に焦点を当て、難民や国内避難民への対応にも目配りし・・・ということで共同スポンサーである他の国連機関を動かしていく仕掛けを積極的に試みている印象です。

 もっとも、今回の会合に関していえば、UNAIDSのサイトには、おざなりな印象はあっても一応、報告が掲載されているのに対し、UNHCR、WFPのサイトでは(少なくとも私は)見つけられませんでした。国連外交の舞台裏ではポスト2015の開発課題をめぐる綱引きが展開されている時期だけに、HIV/エイズとの闘いに対する各機関の熱度(の低さ)は少々、気になります。

 世界中で人道上の緊急事態に直面している何百万もの人のニーズに対応するうえでも、2030年のエイズ流行終結という目標には重要な意味がある。世界の専門家が3月3日、スイスのジュネーブで開かれた会合でこの点に合意した。

 人道上の危機のもとでHIVに対応するには新たな手法が必要なことから、今後5年間のHIV高速対応の中では、危機に直面する人たち特有のニーズをまず把握しなければならない。会合では専門家らがそうしたニーズの検証を行った。

 紛争などで住み慣れた土地からの移住を強いられている難民、避難民は2013年末現在、世界で約5100万人に達している。第二次大戦後、最も多い人数だ。人道にかかわる災害に直面しながらも地元にとどまり、厳しい環境にさらされている人も多い。

 HIVと人道上の緊急事態との関係は複雑だ。たとえば性的な暴力にさらされたり、生活のために性を取引きしなければならなかったりする機会が増えれば、HIV感染の可能性も高まる。加えて保健サービスやHIV感染予防プログラムの提供が困難になり、まったく提供できなくなってしまうこともある。

 国連合同エイズ計画(UNAIDS)、国連難民高等弁務官事務所UNHCR)、世界食糧計画(WFP)が共催した専門家会合では、人道危機に影響を受けている人たちがHIVプログラムを利用できるようにするための最新のエビデンスに基づく知識や経験、データなどが共有された。その成果は、危機におけるHIV対策に焦点をあてる2015年7月の第36回UNAIDSプログラム調整理事会で重要課題として取り上げられることになる。

 議論の対象には以下の分野が含まれている:緊急事態におけるHIV対策の阻害要因;ギャップに対する認識と対処法;組織間の調整;治療継続;さまざまな緊急事態におけるHIV関連の対応策。

HIV and humanitarian emergencies

Addressing the needs of the millions of people around the world affected by humanitarian emergencies will be critical to ending the AIDS epidemic by 2030, top experts agreed at a meeting held in Geneva, Switzerland, on 3 March.

Calling for a new way of addressing HIV in humanitarian emergencies, the experts examined ways to ensure that the specific needs of people affected by humanitarian emergencies are taken in to account as the response to HIV is fast-tracked over the next five years.

At the end of 2013, there were more than 51 million people worldwide who had been forcibly displaced, the highest number since the end of the Second World War. Many others facing humanitarian disasters remain at home but suddenly find themselves in fragile environments.

The links between HIV and humanitarian emergencies are complex. Vulnerability to the virus can be heightened because of, for example, greater exposure to sexual violence and more reliance on transactional sex to meet basic needs. In addition, health services and HIV intervention programmes may become much harder to access or even nonexistent.

The experts at the meeting, which was co-hosted by UNAIDS, the Office of the United Nations High Commissioner for Refugees and the World Food Programme, shared the latest evidence-informed insights, knowledge, experiences and data that support the inclusion of people affected by humanitarian emergencies in HIV programmes. These will be key elements for the upcoming 36th meeting of the UNAIDS Programme Coordinating Board in July 2015 which will have a special focus on HIV in emergency contexts.

Specific areas discussed included: obstacles to HIV programming in emergencies; identifying gaps and how to address them; coordination arrangements between different agencies; improving adherence to treatment; and tailoring HIV-related programmes to address a range of differing types of emergency.
