鳴かないホトトギスを前にして、殺すのでも待つのでもなく、鳴かせてみようといったのは戦国のどの指導者だったでしょうか。米国のトランプ政権はHIV感染のPrEP（曝露前予防）という予防手段の普及拡大をはかるため、感染リスクの高い人を対象にしたPrEP薬の無償配布プログラム『Ready, Set, PrEP』を発表しました。

Ready, Set, Goは日本語に訳すと「よ～い、どん」といった感じです。新たなプログラムは「Go」の部分が、「PrEP」になっているので、「PrEPいざ発進」あたりでしょうか。
HIV感染の高いリスクに曝されている人で、HIV検査が陰性、医師からPrEP薬のきちんとした処方箋を受けている、しかも医療保険でPrEP薬がカバーされていない人が対象になります。
米国政府のHIV/エイズ啓発サイトHIV.govにも『Ready, Set, PrEP』の紹介ページがあります。

今後10年間に推定で毎年20万人がこのプログラムの恩恵をうけるということです。計算の仕方がよく分かりませんが、毎年20万人が毎日服用するPrEP薬を無償で支給されるということだと、毎年20万人で10年間だから計200万人と計算したくなりますが、1年でやめてしまっては意味がないので、同じ20万人が10年間ずっとPrEPを続けているだけのような気もします。
それでも大変な金額ではありますが、個人の切実な問題とは別に、マクロで見たときの予防効果としてはどうなんでしょうね。解説していただける方がいらしたら、よろしくお願いします。
トランプ政権の予防策はPrEPにかなり前のめりな感じを個人的には受けます。ただし、これがHIV/エイズに携わる（日本も含めた）医療関係者の考え方の趨勢なのかもしれませんね。私は予防の選択肢の一つとして、希望する人がいれば使えるような条件を整えておくという程度に考えておいた方がいいのではないかと思っています。
ホトトギスの例に倣えば、鳴くまで待とうの家康派といいますか。待ち方の問題もあるので、ニーズに対しては応じられるようにして待つというかたちだとすると、日本ではどんな方法が考えられるのか。今年は少しいろいろな立場の人の考え方や意見を聞いてみる必要がありそうです。

Ready, Set, PrEP Expands Access to HIV Prevention Medications for Thousands of People
Although more than one million people who are at risk for HIV in the United States could benefit from pre-exposure prophylaxis (PrEP) medications, only a small fraction receive it. The U.S. Department of Health and Human Services (HHS) launched Ready, Set, PrEP , a nationwide program that makes PrEP medications available at no cost to individuals who lack prescription drug coverage.
Why Should I Take PrEP Medication?
PrEP medications allows those at risk for HIV to take control of their health and reduce their risk. If you are at risk of HIV exposure, ask a healthcare professional if PrEP medication is right for you. PrEP medications only protect against HIV, so condoms are still important to prevent other sexually transmitted infections.
How Does It Work?
PrEP medications are safe and effective and can help prevent HIV. When taken prior to an exposure, PrEP medications can work to prevent HIV.
Do I Qualify?
The Ready, Set, PrEP program provides access to PrEP medications at no cost to people who qualify. To receive PrEP through the program, you must:
1 Test negative for HIV;
2 Have a valid prescription from your healthcare provider; and
3 Not have health insurance coverage for outpatient prescription drugs.
If you receive PrEP medication through the Ready, Set, PrEP program, you will not have to pay for the medicine. The costs of clinic visits and lab test costs may vary depending on your income.
How to Enroll
・Before enrolling in the Ready, Set, PrEP program, talk to a healthcare provider to see if PrEP medication is right for you and obtain a prescription. If you do not have a provider, you can find one at locator.hiv.gov.
・Once you have the necessary prescription, visit GetYourPrEP.com or call toll-free 855-447-8410 to qualify and enroll. When completing the enrollment form, you must include the name of the healthcare provider who wrote your prescription.
・If you have any problems enrolling, call 855-447-8410 or ask for assistance in-person at a healthcare provider’s office, including a community health center, where trained staff can provide support.
HIV Prevention in Action
Ready, Set, PrEP will make PrEP medications available nationwide, and is a key component of the Ending the HIV Epidemic: A Plan for America (EHE) initiative. EHE aims to reduce the number of new HIV transmissions in the United States by 75% in five years and by 90% in 10 years.
Over the next ten years, it is projected that more than 200,000 people per year will benefit from receiving PrEP medications through this program. Ensuring people and communities have access to PrEP medications will keep people healthy and brings us all closer to a future without HIV.
