米大統領エイズ救済緊急計画（PEPFAR）は単一の疾病に対し、一カ国による世界で最大の資金貢献である。米国政府はPEPFARを通じ、オバマ政権下における500億ドルを含め総額650億ドルの投資を行っている。国際社会はHIV/エイズの流行を拡大から縮小へと転じつつある。それでもまだ、完遂への道のりは遠い。流行を制御するにはこれまでの成果を持続し、拡大させていかなければならない。
PEPFARの新たな予防、治療目標
発足以来、PEPFARは野心的な目標を掲げてきた。その成果は何百万というHIV新規感染を防ぎ、何百万人もの命を救い、エイズから自由な世代の実現に向けた動きを大きく前進させてきた。
オバマ大統領はPEPFARの新たなHIV予防と治療のターゲットを設定し、プログラムをさらに果敢に推し進めようとしている。
予防ターゲット
・PEPFARは初めて、十代の少女と若い女性の新規HIV感染予防のための具体的ターゲットを設定した。パートナー国やグローバルファンド、民間部門と協力し、

治療ターゲット
・2016年末までにPEPFARは計1140万人の子供、HIV母子感染予防オプションB+を受けている妊婦、および成人が抗レトロウイルス治療を受けられるよう支援する。このうち720万人はPEPFAR資金による直接支援、420万人にはパートナー国と協力して技術支援を提供する。
・2017年末までにPEPFARは計1290万人の子供、HIV母子感染予防オプションB+を受けている妊婦、および成人が抗レトロウイルス治療を受けられるよう支援する。このうち850万人はPEPFAR資金による直接支援、440万人にはパートナー国と協力して技術支援を提供する。
・世界エイズ・結核・マラリア対策基金（グローバルファンド）およびパートナー国の資金も含め、2017年までに協力して1850万人の男女および子供に生命を救う治療を提供できるであろう。
10代の少女および若い女性が夢を実現できるよう支援
HIV/エイズは低・中所得国における出産可能な年齢層の女性、少女の間では、最も大きな死亡原因となっている。年間で38万人の若い女性、少女がHIVに新規感染している。1週間あたり7300人、1日平均1000人以上である。サハラ以南のアフリカでは、男性パートナーよりHIV陽性率が5倍も高い。
この状態は変えなければならない。2014年世界エイズデーにPEPFARはビル＆メリンダ・ゲイツ財団、ガール・エフェクトと協力して、エイズの流行の影響が大きいサハラ以南のアフリカの10カ国 （ケニア、レソト、マラウィ、モザンビーク、南アフリカ、スワジランド、タンザニア、ウガンダ、ザンビア、ジンバブエ）でDREAMSパートナーシップを開始した。若い女性、少女が自分の意思で、はつらつとして意欲的な生活、良き助言者に恵まれ、安全でエイズから自由な生活を送れるようにするための計画だ。2014年には上記10カ国が世界全体の若い女性、少女の新規HIV感染者のほぼ半分を占めていた。
米国政府はPEPFARを通じ、若い女性、少女たちが将来、エイズから自由な世代になることができるようほぼ5億ドルの投資を行うことにしている。その中には、予防対策およびその関連分野への戦略的提携策としてPEPFAR主導のDREAMSパートナーシップに3億ドルの追加的な投資を行うことも含まれている。この投資はPEPFARと民間のパートナーが2014年世界エイズデーに明らかにしたDREAMSへの約束に基づくものだ。

The U.S. President’s Emergency Plan for AIDS Relief (PEPFAR) is the largest commitment by any nation to address a single disease. Through PEPFAR, the U.S. government has invested $65 billion dollars, including nearly $50 billion since the beginning of the Obama administration. As a global community, we have begun to reverse the HIV/AIDS epidemic. Yet, our work is far from done. To control the epidemic and, ultimately, achieve an AIDS-free generation, we must sustain and expand our collective gains.
New PEPFAR HIV Prevention and Treatment Targets
Since its inception, PEPFAR has set ambitious targets – the achievement of which has prevented millions of new HIV infections, saved millions of lives, and accelerated progress toward achieving an AIDS-free generation.
President Obama is setting new PEPFAR HIV prevention and treatment targets, launching a bold course for the program moving forward.
Prevention Targets;
・For the first time ever, PEPFAR has set specific targets for preventing new HIV infections among adolescent girls and young women. Working jointly with partner countries, the Global Fund, and the private sector,
PEPFAR will:
O By the end of 2016, achieve a 25 percent reduction in HIV incidence among adolescent girls and young women (aged 15-24) within the highest burden geographic areas of 10 sub-Saharan African countries.
O By the end of 2017, achieve a 40 percent reduction in HIV incidence among adolescent girls and young women (aged 15-24) within the highest burden geographic areas of 10 sub-Saharan African countries.
・By the end of 2016, PEPFAR will provide 11 million voluntary medical male circumcisions for HIV prevention, cumulatively.
・By the end of 2017, PEPFAR will provide 13 million voluntary medical male circumcisions for HIV prevention, cumulatively.
Treatment Targets:
・By the end of 2016, PEPFAR will support a total of 11.4 million children, pregnant women receiving B+, and adults on life-saving anti-retroviral treatment, of which 7.2 million are directly supported by PEPFAR funding and 4.2 million are supported through technical assistance, jointly with partner countries.
・By the end of 2017, PEPFAR will support a total of 12.9 million children, pregnant women receiving B+, and adults on lif-saving anti-retroviral treatment, of which 8.5 million are directly supported by PEPFAR funding and 4.4 million are supported through technical assistance, jointly with partner countries.
・With the inclusion of Global Fund to Fight AIDS, Tuberculosis and Malaria (Global Fund) and partner country resources, we will be able to jointly support 18.5 million men, women, and children on life-saving treatment by the end of 2017.
Supporting Adolescent Girls and Young Women to Fulfill their DREAMS
HIV/AIDS is the leading cause of death for women and girls of reproductive age in low-and middle-income countries. Every year, 380,000 adolescent girls and young women are infected with HIV—7,300 every week, over 1,000 every day. In sub-Saharan Africa, they are up to five times more likely than their male counterparts to be living with HIV/AIDS.
This must change. On World AIDS Day 2014, PEPFAR joined with the Bill & Melinda Gates Foundation and Girl Effect to launch the DREAMS Partnership in 10 high-burden countries in sub-Saharan Africa (Kenya, Lesotho, Malawi, Mozambique, South Africa, Swaziland, Tanzania, Uganda, Zambia, and Zimbabwe), to ensure that adolescent girls and young women have an opportunity to live Determined, Resilient, Empowered, AIDS-free, Mentored, and Safe lives. These 10 countries account for nearly half of all the new HIV infections that occurred among adolescent girls and young women globally in 2014.
The U.S. government, through PEPFAR, is now investing nearly half a billion dollars to support an AIDS-free future for adolescent girls and young women. This includes strategically aligning $300 million in additional prevention investments in support of the PEPFAR-led DREAMS partnership and related efforts. This investment builds on what PEPFAR and private partners committed to DREAMS on World AIDS Day 2014.
